By scheduling and receiving any waxing or spa services at Evolve Wax Spa, you acknowledge that body waxing and related aesthetic treatments carry inherent risks, including but not limited to skin irritation, redness, swelling, bruising, and allergic reactions, and increase histamine response. You voluntarily assume all risks associated with these services.

Evolve Wax Spa, its owners, employees, and independent contractors shall not be held liable for any injury, adverse reaction, damage, or loss resulting from services rendered, unless caused by gross negligence or willful misconduct.

Clients are responsible for disclosing all relevant medical conditions, medications (including but not limited to Accutane, Retin-A, retinoids, blood thinners, or topical steroids), allergies, skin sensitivities, hormone therapy, and any recent cosmetic procedures prior to receiving services. Failure to disclose pertinent health information releases Evolve Wax Spa from any liability related to adverse reactions or complications.

Age Requirement & Minor Consent

Clients under the age of **18** must have a parent or legal guardian present to provide verbal consent prior to receiving services.

Right to Refuse Service

Evolve Wax Spa reserves the right to refuse or discontinue service to any client at any time for reasons including but not limited to: suspected contraindications, inappropriate behavior, intoxication, unsanitary conditions, or the safety of our staff and clients.

These terms and conditions shall be governed by and construed in accordance with the laws of the State of Florida, without regard to conflict of lnstitute.
